Certified Nurse Assistant (CNA) Home Care

We are currently seeking a Home Health Aide (HHA) or Certified Nurse Assistant (CNA) to join our Home Care and Private Duty Services team. In this role, you will be a vital part of a patient-centered care team, providing support to patients in their homes while collaborating with families and interdisciplinary healthcare providers to ensure high-quality care and positive outcomes.

Key Responsibilities:

• Provide direct patient care in the home setting under the direction of the care team

• Assist patients with activities of daily living and personal care needs

• Communicate effectively with patients, families, and healthcare team members

• Monitor and report changes in patient condition or progress

• Maintain accurate documentation of care provided

• Ensure patient safety and follow all care plans and protocols

QUALIFICATIONS

Education:

• High school diploma or equivalent required

Experience:

• Minimum of 1 year of healthcare experience preferred

Knowledge & Skills:

• Ability to work independently and take initiative

• Strong time management and organizational skills

• Demonstrates accountability and responsibility for safe patient care

• Effective communication and interpersonal skills

Licensure, Registration, or Certification:

• Current Certified Nurse Aide (CNA) or Home Health Aide (HHA) licensure in the state of Kansas

• CNA candidates must be willing to complete Home Health Aide course and testing

• Current CPR/Basic Life Support (BLS) certification required

• Reliable transportation and valid driver’s license required
