Care Coordination Assistant
The Care Coordination Assistant reports to the Executive Director of Clinical Outcomes and presents a professional image to all customers, patients, physicians, and staff. This position provides administrative and clerical support to the Case Management Department by coordinating communication, supporting daily operations, and assisting with patient care coordination activities.
The Care Coordination Assistant is expected to work independently at times, exercise sound judgment, and perform duties without direct observational supervision.
Key Responsibilities

Provide clerical and administrative support to the Case Management Department

Coordinate communication with patients, physicians, and interdisciplinary team members

Assist with efficient clinic and reception operations related to care coordination

Maintain confidentiality and comply with HIPAA and departmental standards
Qualifications
Education
-
High school diploma or equivalent required
Experience
-
Previous experience in a healthcare setting required
Knowledge & Skills

Strong interpersonal skills with a focus on courteous, professional communication

Proficiency with computer keyboarding and basic computer applications

Effective verbal and written communication skills

Strong organizational skills with the ability to multitask and prioritize

Knowledge of basic medical and clinical terminology preferred

Ability to work efficiently under pressure and take direction

Demonstrated professionalism, reliability, and sound judgment

Ability to comply with HIPAA guidelines and confidentiality requirements
