Clinic Coordinator RN/LPN

The Clinic RN/LPN Coordinator is a professional nurse who works in collaboration with the provider and care team to ensure high-quality, patient-centered care in our Family Medicine Clinic. This position supports Medicare Wellness visits, coordinates care for patients recently discharged from the hospital or emergency department, and supports the day-to-day operations of the clinic.
This role is ideal for someone passionate about patient outcomes, driven by quality, and adaptable in a fast-paced environment. The ideal candidate values strong communication, clinical competence, and compassionate care.
This position reports directly to the Clinic Manager.
Essential Responsibilities:

Collaborate with physicians and the care team to coordinate and deliver patient care

Conduct Medicare Wellness visits and provide follow-up for patients post-discharge

Support clinic operations and ensure clinical workflows support optimal patient outcomes

Utilize the nursing process and critical thinking to assess, plan, implement, and evaluate patient care

Demonstrate accountability, initiative, and the ability to prioritize and multitask in a clinic setting

Foster strong, respectful relationships with patients and colleagues

Contribute to a team-based care model and participate in continuous improvement initiatives

Maintain patient documentation in the electronic health record (EHR) accurately and timely
Qualifications:
Education and Experience:

Graduate of an accredited school of nursing (ACEN or CCNE required)

Experience in clinic care and Medicare Wellness responsibilities strongly preferred
Licensure, Registration, Certification:
-
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure in the State of Kansas

Current CPR/Basic Life Support (BLS) certification

Advanced Cardiac Life Support (ACLS) certification preferred
Knowledge, Skills & Abilities:

Knowledge of age-appropriate care and the normal signs of aging

Strong interpersonal and communication skills

Computer competency including EHR documentation

Ability to manage time efficiently and work both independently and within a team
